Application for Charity assistance

Charity Financial assistance will be provided to individual(s) and families, who are really in dire needs.

Our organization will try to keep you afloat during unfavorable times. If you are going through

financial hardships.

Please provide the following documents:

For all adults living in the household.

For everyone in the household.

Such as current lease, 2 most recent bank statements, pay-stubs, W2s,

1099s, most recent tax returns.

Please provide letter from employment commission.

If you receive public assistance of any kind such as: TANF, SSI, Section 8, etc. Please provide relevant letter.

In order to determine your eligibility, please provide complete list of documents requested

above.

In a written statement below, please provide the hardship and requested financial assistance.
